Comprehensive Assessment Clinic

Providing a thorough evaluation of your child’s needs to customize their care team and treatment

Our goal is to develop a complete, individualized care plan for your child, addressing their specific needs as well as your goals for your child’s health. That’s why care for children with complex conditions at DMG Children’s Rehabilitative Services (DMG CRS) starts with our Comprehensive Assessment Clinic (CAC).

Through the CAC, an extensive evaluation of your child’s needs is completed to determine what medical specialists will be on your child’s care team, as well as which experts from other therapies and services will partner with you and your child’s physicians to support your child throughout treatment.

The CAC is staffed with pediatric nurse practitioners who will meet you and your child for this important first visit to DMG CRS. During this visit, your child will be examined from head to toe, and all medical records will be reviewed. In addition, this visit includes in-depth conversations between you and DMG CRS staff to ensure a clear understanding of your child’s needs and address any questions or concerns you may have.

Once the comprehensive assessment is completed, referrals to the appropriate DMG CRS pediatric specialists, therapies and support services will be made, and DMG CRS will work with you to schedule appointments. Whenever possible, multiple appointments will be made for the same day to save you and your family time and optimize care coordination. We will:

You will also be given a packet of information to take home, which will include your child’s care plan, a copy of medical referrals made, a list of scheduled appointments, and telephone numbers for any questions or concerns.

Our goal is for you to leave your child’s CAC appointment confident in DMG CRS’ ability to provide the best individualized healthcare for your child with special needs.
