How can I find a specific DMG CRS specialist?
DMG CRS includes pediatric specialists in more than 25 medical specialties. To search for a provider by name, use the Provider Search tool and input the provider’s first and/or last name. To search for a provider in a specific medical specialty, select the specific pediatric medical specialty to see a list of the providers.
Does DMG CRS only serve CRS patients?
No. DMG CRS provides care to pediatric patients of all ages across a wide range of health conditions. Whether your child needs an annual exam or coordinated care for a complex condition, DMG CRS is here to support the health of your child.
How do I schedule an appointment for my child with a DMG CRS medical provider?
Whether you are a new or existing patient, please contact the DMG CRS at (602) 914-1520 to schedule an appointment. Established DMG CRS patients can also request an appointment through the DMG Patient Portal.
Does DMG CRS only serve AHCCCS patients?
No. DMG CRS provides care to pediatric patients of all ages and accepts AHCCCS as well as most commercial medical insurances and cash-pay.
I need to speak to my child’s DMG CRS medical provider. How do I contact him/her?
To contact your DMG medical provider, please call DMG CRS at (602) 914-1520 and ask for the provider or his/her medical specialty clinic. Please be aware that the medical provider will not likely be able to take your phone call immediately as he/she is serving patients. However, the medical receptionist will either take a message and provide it to your DMG CRS physician or have another qualified team member speak with you to answer your questions and/or address your concerns as quickly as possible. You may also use the secure, confidential Patient Portal to ask questions, share information or message your child’s DMG CRS medical provider.
Does DMG CRS have bilingual staff or translators?
Yes. Many DMG CRS staff members are bilingual and speak English and Spanish, including some social workers. In addition, DMG CRS provides translation services across many languages for families who do not speak English or are more comfortable communicating in their primary language.

How can I pay my DMG bill? Can I pay it online? What payment methods are accepted?
For your convenience, we offer a few options to pay your DMG bill. Having your DMG billing statement available when paying your bill will save you time.
- Online with a credit or debit card
- By phone with a credit or debit card: Call DMG’s billing department at (602) 470-5075 Monday-Friday, 8am-5pm AZ time.
- By mail to the address noted on your DMG billing statement (Lockbox 860183, Minneapolis, MN 55486-0183). Please do not send cash if paying by mail.
- In-person at DMG billing office, 2929 E. Thomas Rd., Phoenix, AZ 85016. Cash, credit or debit card, personal check or money orders are accepted.
What health insurance plans does DMG CRS accept?
All DMG locations, including DMG CRS, accept most major insurance plans, including Medicare and AHCCCS. To find out if DMG CRS is contracted with your health insurance plan, please call (602) 914-1520.
I received a bill for medical services provided by a DMG physician, but I don’t know what it’s for and/or recognize it. How can I get more information? What should I do?

How do I request my child’s medical records?
To get copies of your child’s medical records sent to you or another provider’s office outside of DMG, please contact DMG medical records at (602) 914-1520 extension 2.
