Our goal is to develop a comprehensive care plan for your child, addressing his or her specific needs as well as your goals for your child’s health. That’s why DMG CRS includes a Comprehensive Assessment Clinic (CAC) as the first step in your child’s treatment at our multi-specialty clinic. Through the CAC, a thorough needs evaluation will be completed to determine what medical specialists will be on your child’s care team, as well as which experts from other support services such as occupational and/or physical therapy, child life, and more will collaborate with your child’s physicians and you on treatment.
The CAC is staffed with pediatric nurse practitioners who will meet you and your child for this very important first visit to DMG CRS. During this visit, your child is examined from head to toe and all medical records are reviewed. In addition, this visit includes in-depth conversations between you and DMG CRS staff to ensure a clear understanding of your child’s needs and address any questions or concerns you may have.
Once the assessment is complete, referrals to the appropriate DMG CRS pediatric specialists and support services are made, and DMG CRS works with you to schedule appointments. Whenever possible, multiple appointments are made for the same day to save you and your family time and optimize care coordination. In addition, the CAC staff can help you coordinate transportation to and from appointments at DMG CRS, if needed; will follow up to ensure medical appointments were kept; and will engage a patient advocate to help you and your child with school concerns or connect you with a social worker. You will also be given a packet of information to take home, which will include your child’s care plan, a copy of medical referrals made, a list of scheduled appointments, and telephone numbers for any questions or concerns.
